The COVID-19 Vaccination for Children 5-11 years old (1st and 2nd dose)

Group vaccinaion schedule for February is provided. Reservation can be acceptable if you have the vaccination ticket. Reception hours differ depending on how to make your reservation. Reservations can be made up to Sunday three weeks after every Monday.
Children who will be 12 years old by February 1, 2023 can't make a reservation　by LINE app, so please call Zama COVID-19 Vaccination Call Center (0120-115-875).

Please come with a parent / guardian

Children must be accompanied by a parent / guardian. If they can't come, the relatives who know the health condition of the children can accompany.
Please be sure to fill out the Prevaccination screening questionnaire with 1. Parental signature on the consent section and 2. Emergency contact phone number.

About the vaccination

Eligible people

Children 5-11 years old who are residens of Zama City.

Vaccination is optional

• Please get vaccinated of your own decision or parent's/guardian's decision understanding both the effectiveness in preventing infectious diseases and the risk of side effects.
• The vaccination for the children 5-11 years old will be given with the parental (guardian's) consent. Parent/guardian needs to sign their name on prevaccination screening questionnaire for the vaccination.
• Please do not force anyone to be vaccinated, and do not discriminate against those who have not been vaccinated.

Children with underlying medical conditions who may be at a higher risk for severe illness from COVID-19 are encouraged to get the vaccine.

Can COVID-19 vaccine be given at the same time as other vaccines?

COVID-19 vaccine and influenza vaccine can be given at the same time. If you receive those vaccines at the different medical institutions on the same day, it will be done at the doctor's discretion.

【Exsample】
1.COVID-19 vaccination and influenza vaccination at the same time at the same clinic
→　Possible
2.COVID-19 vaccination and influenza vaccination on the same day but different clinic
→　will be done at the doctor's discretion
3.Influenza vaccination at a clinic and COVID-19 vaccination at the group vaccination site on the same day
→　Impossible

※2weeks (13days) before and after of your COVID-19 vaccination date, you can NOT receive other vaccine except the influenza vaccine.

Q　What are the effectiveness of COVID-19 vaccine?

The vaccine stimulates your body’s immune system to produce antibodies and other proteins that will fight the virus if you’re exposed to it. This can reduce the risk of getting infected, and prevent the serious symptoms if you caught COVID-19.

Q　What kind of symptoms possibly cause as side effects?

There is a possibility that you may have some symptoms of side effects after a few days of vaccination. Most common side effect is the pain at the injected site. It may appear on the next day. The report says the tiredness and fever are more common after the 2nd dose than after the 1st dose. Most side effects are mild or moderate.

About the myocarditis after the vaccination

Myocardites is inflammation of the heart muscle. Please seek medical care if your child have symptoms, such as, chest pain, rapid heartbeats, shortness of breath and swelling within 4 days after the vaccination.

Procedure of the vaccination

1. Vaccination ticket will be sent

The vaccination ticket will be sent sequentially a week after 5th birthday. If the vaccination ticket haven't arrived 2 weeks after 5th birthday, please call Zama COVID-19 Vaccination Call Center (0120-115-875).The set of vaccination ticket and notification will be sent with the light blue color envelope.

2. Make a reservation

Please see "How to make a reservation".

3. Receive vaccination

Children who receive the vaccination must be accompanied by a parent or gardian.
Please wear the shirt which can show your shoulder easily.

Things you need to bring

1	The set of vaccination ticket sent from Zama City	Please do NOT peel off the seal on the vaccination ticket.
2	Prevaccination Screening Questionnaire	Please fill out the Questionnaire. Please do NOT use pencil or erasable ballpoint pen.
3	Identity verification card/document	Ex: Health insurance card etc
4	Parent's / Gardian's Identity verification card/document	Ex: Driver's license, Health insurance card etc
5	Mother and child health hand book	To check the vaccination interval of other vaccinations
6	Prescription record	Only the person who has it
7	Ballpoint pen	Please bring yours if you don't want to use the　prepared at the venue.
8	Body thermometer	Please bring yours if you don't want to use the　prepared at the venue.
9	Certification of accepting the application form to receive the vaccination outside of registered city	Only the person who apply to receive the vaccination outside of registered city

How to make a reservation

Children who will be 12 years old by February 1, 2023 can not make a reservation throuh LINE app, so please call Zama COVID-19 Vaccination Call Cernter (0120-115-875).

Making a reservation using LINE app

Hours：24 hours (except the maintenance hours)
Please install LINE app and add " Zama City official LINE account " to your friends list.
You can't make a reservation through the computer. When you use iPad, please get a LINE account for iPad.

How to change / cancel your reservation

• To avoid the double -booking, please cancel the reservation when you made reservations at 2 or more vaccination sites.
• To change the date or time of your vaccination, you need to make a reservation again after you cancel your first reservation.

Using LINE：You can change or cancel your reservation by the device you used to make your reservation.
You can't cancel through LINE after 07:00p.m. of the day before of your vaccination date.
