Issuing of COVID-19 Vaccination Ticket(1st and 2nd dose)

You need a "Vaccination Ticket" to receive the　COVID-19 vaccination.
If you moved to Zama city, or lost the vaccination ticket / don't receive it, you need to apply for the vaccination ticket.

Which case can you apply for the ticket?

• People who moved to Zama city after June 25, 2021 and want to take 1st and 2nd dose
• People who moved to Zama city after December 12, 2021 and want to take 3rd dose
• Children 5-11 years old who moved to Zama city after February 16, 2022 and want to take 1st and 2nd dose
• Lost ticket / Ticket was ripped / Ticket got damaged
• Haven't received ticket
• There is a printing mistake on the ticket
• Other reasons you need to apply

How to receive ticket

• The vaccination ticket will be sent by mail (post)　in about a week after you apply.
• For people who want to take 3rd dose, the vaccination tickt will be sent about a week after you apply or about 6 months after your 2nd vaccination, whichever comes first.

• ※If you are in a hurry, please contact city official of New Coronavirus Vaccination Promotion Division before you apply.
• ※You can't get the vaccination ticket on the day you apply.

Notes

• When you have moved out from Zama city, the vaccination ticket issued by Zama city will not be able to use after you transfer your certificate of residence to the city you moved in.
• If you have moved out from Zama city, and you want to take the vaccination in the city you moved in, please contact the city official of the city you moved in.
