Eligible people
People 12 years and older who have had 1st and 2nd doses of COVID-19 vaccination more than 3 months ago
The vaccination targeting the Omicron Variant is provided one time per person.
The schedule of sending the vaccination ticket
The vaccination ticket will be sent for people 12 years and older who have had 1st and 2nd doses of their COVID-19 vaccination more than 3 months ago. If you haven't receive the vaccination ticket 14 days after your scheduled date of sending vaccination ticket, please call Zama COVID-19 Vaccination Call Center (0120-115-875). If you find any errors on it, please correct it through the LINE app, or please call Zama COVID-19 Vaccination Call Center (0120-115-875).

About the type of vaccines targeting the Omicron Variant provided at the vaccination are as follows;

※Eligibility age of Moderna vaccine is changed to 12 years and older.

About the vaccine targeting the Omicron Variant (BA.1, BA. 4/5)
The report says that the booster shot with the vaccine targeting the Omicron Variant can help to protect from getting seriously ill with COVID-19 more than the vaccine before and can help to improve people's protection from getting infected. It also says that there is a highly possibility that mixing and matching the COVID-19 vaccines including the vaccine targeting the Omicron Variant is effective against other variants.

Can COVID-19 vaccine be given at the same time as other vaccines?
COVID-19 vaccine and influenza vaccine can be given at the same time. When you receive those vaccines at the different medical institutions on the same day, please talk with the doctor.

→ Possible
(2) COVID-19 vaccination and influenza vaccination on the same day but at the different clinic
→ Please talk with the doctor
(3) Influenza vaccination at a clinic and COVID-19 vaccination at the group vaccination site on the same day
→ Impossible
※2weeks (13days) before and after of your COVID-19 vaccination date, you can NOT receive other vaccine except the influenza vaccine.
Notes for persons 12-15 years old
Children under 15 years old must be accompanied by a parent/guardian. If they can't come, the relatives who know the health conditions of the children can accompany. Please be sure to fill out the Prevaccination screening questionnaire with 1. Parent's signature on the consent section and 2. Emergency contact phone number.
Parking place of Sky Arena Zama
The parking lot of the Sky Arena Zama will be shared with the Sky Green Park and become paid parking, but the parking fee will be free when you park for the vaccination. Please bring the parking card to the vaccination site. Thank you for your cooperation.
Zama emergency supplies storehouse area near the Sky Arena Zama will be opened as the parking place until 05:00p.m. (Midorigaoka 6-1-15).

The parking place of Zama city offcce might be full when the Harmony Hall Zama has an event. Please use public transportation or use the paid parking lot near by.
About the air-conditioner
The temperature of the air-conditioner differs depending on the vaccination site, so bring your clothes that you can adjust to the temperature.
How to make a reservation
- You can make a reservation when you receive your vaccination ticket. The reservation are available until 07:00p.m. on the day before each vaccination date, unless they are fully booked.
- You can set a reservation date until Sunday 3 weeks after every Monday.
- People who will turn 18 years old will be able to make a reservation using LINE app from 1 week after your 18th birthday. Please call Zama COVID-19 Vaccination Call Center (0120-115-875) if you want to make your reservation from your 18th birthday.

How to change / cancel your reservation
- To avoid the double -booking, please cancel the reservation when you made reservations at 2 or more vaccination sites.
- To change the date or time of your vaccination, you need to make a reservation again after you cancel your first reservation.
Using LINE：You can change or cancel your reservation by the device you used to make your reservation.
You can't cancel through LINE after 07:00p.m. of the day before of your vaccination date.
