About the vaccination
Vaccination is optional
Please get vaccinated of parent's / guardian's decision understanding both the effectiveness in preventing infectious diseases and the risk of side effects. The vaccination will be given with the parental (guardian's) consent. Parent / guardian needs to sign their name on prevaccination screening questionnaire for the vaccination.
Please do not force anyone to be vaccinated, and do not discriminate against those who have't been vaccinated.
Vaccination interval
The vaccination will be 3 times in total.
The interval between the 1st and 2nd dose is 20 days.
The interval between the 2nd and 3rd dose is more than 55 days.
Schedule of sending the vaccination ticket
The vaccination ticket has been sent on November 2, 2022, and it will be sent to the infant who become 6 months old.

Vaccination period
COVID-19 vaccine can be received through the end of March 2023. Depending on the 1st vaccination date, you may not be able to complete 3 vaccinations by March 31, 2023. Please keep this in mind when considering the vaccination.
About the vaccine
Pfizer vaccine (for 6 months - 4 years old)
Can COVID-19 vaccine be given at the same time as other vaccines?
COVID-19 vaccine and influenza vaccine can be given at the same time. If you receive those vaccines at the different medical institutions on the same day, it will be done at the doctor's discretion.

- COVID-19 vaccination and influenza vaccination at the same time at the same clinic
→ Possible - COVID-19 vaccination and influenza vaccination on the same day but different clinic
→ will be done at the doctor's discretion - Influenza vaccination at a clinic and COVID-19 vaccination at the group vaccination site on the same day
→ Impossible
※2weeks (13days) before and after of your COVID-19 vaccination date, you can NOT receive other vaccine except the influenza vaccine.

Clothes you need to wear
Please wear the shirt which can show your shoulder easily.
The temperature of the air-conditioner differs depending on the vaccination site, so bring your clothes that you can adjust to the temperature.
1st and 2nd dose
Eligible people
Children 6 months - 4 years old at the time of the 1st dose and who are residents of Zama City
※When the children receive the 1st dose after the day before 5th birthday, the vaccine will be the one for 5 - 11 years old. For more information, please click the link below.
Children must be accompanied by a parent / guardian. If they can't come, relatives who know health condition of the child can accompany. Please be sure to fill out the Prevaccination screening questionnaire with 1. Parental signiture on the consent section and 2. Emergency contact phone number.

About the vaccination
Eligible people
Children 6 months - 4 years old who have received 2nd dose more than 55 days ago.
※Even if the child turns 5 years old in the middle of a total of three vaccinations, he/she must get the same vaccine (6 months - 4 years old) for 2nd dose and 3rd doses.
Children must be accompanied by a parent / guardian. If they can't come, relatives who know health condition of the child can accompany. Please be sure to fill out the Prevaccination screening questionnaire with 1. Parental signiture on the consent section and 2. Emergency contact phone number.

※Reservation for 1st and 2nd doses can not be made by LINE app, so please call Zama COVID-19 Vaccination Call Cernter (0120-115-875).
※Your reservation will be deleted when you make 3rd dose reservation setting the date before 56 days after 2nd dose.
- You can make a reservation when you receive your vaccination ticket.
- The reservation is available until each cancellation date, unless they are fully booked.
- Reservation can be made up to Sunday 3 weeks after.
- Children who will be 5 years old by March 1, 2023 can not make a reservation using LINE app, so please call Zama COVID-19 Vaccination Call Cernter (0120-115-875).

How to change / cancel your reservation
- To avoid the double -booking, please cancel the reservation when you made reservations at 2 or more vaccination sites.
- To change the date or time of your vaccination, you need to make a reservation again after you cancel your first reservation.
