Minimum age for receiving 1st and 2nd dose is lowered to 12 years old at the vaccination site of Rembrandt Hotel Ebina run by Kanagawa prefecture.

About the vaccination

Procedure of  the vaccination

1

 Receive the vaccination ticket

 We have sent the vaccination ticket to the registered residence address.
 Person who turns 12 years old can still use the children's vaccination ticket sent from Zama city. 
Please click here if you just moved to Zama city or if you lost your vaccination ticket.

 Choose the type of vaccination

 There are 2 types of vaccination.
 Group vaccination  / Vaccination at the medical institutions

※2nd dose of vaccination is reserved automatically at the same place and same time 3 weeks after the 1st dose.

Things you need to bring

1 The set of vaccination ticket sent from Zama city  Please do NOT peel off the seal on the vaccination ticket.
2 Prevaccination Screening Questionnaire  Please fill out the Questionnaire. Please do NOT use pencil or erasable ballpoint pen.
3 Identity verification card/document

 Ex: Residence card, Driver's license
 If you are under 15, a parent/guardian who are accompanied with you for the vaccination needs to bring the Identity verification card/document also. 

4 Prescription record  Only the person who has it.
5 Ballpoint pen  Please bring yours if you don't want to use the ballpoint pen or body thermometer which are prepared at the venue.
6 Body thermometer
7 Certification of accepting the application form to receive the vaccination outside of registered city  Only the person who apply to receive the vaccination outside of registered city

※You can bring your own drink.

 1st dose : September 3rd (Sat)   8:45 a.m.~11:15 a.m. 
 2nd dose: September 24th (Sat)   8:45 a.m.~11:15 a.m.

August 10 (Wed)   From 10:00 a.m.

Children who turned 12 years old can make your reservation 1 week after the 12th birthday through LINE app.
If you are in a hurry, please call Zama COVID-19 Vaccination Call Center (☎0120(115)875) to make your reservation.

To change / cancel your reservation, please click here.
※Your 2nd dose will be reserved automatically at same time 3 weeks after your 1st dose.
※There is no waiting room at the venue so please refrain from coming the venue before your reservation time.
※The Phase 8 vaccination dates will be provided as new infromation becomes available.
Children 12-15 years old must be accompanied by a parent/guardian. If they can't come, the relatives who know the health conditions of the children can accompany. Please be sure to fill out the Prevaccination screening questionnaire with 1.Parental signiture at consent section and 2. emergency contact phone number.

How to change/cancel your reservation

To avoid wasting COVID-19 vaccine, please contact as soon as possible when you want to change / cancel your reservation.
※When you make your reservations at 2 or more vaccination sites, please cancel your reservation except the one you want to receive the vaccination to avoid the double booking.
※To change the date or time of your vaccination, you need to make a reservation again after you cancel your first reservation.

 Cancel by LINE  You can cancel your reservation by the device you used to make your reservation.

 You can't cancel by LINE after 7:00 p.m. of the day before of your reservation date.

※Children 12-15 years old must be accompanied by a parent/guardian. If the medical institution permits, the children who are junior high school student and above can receive the vaccination without accompanying a parent/guardian.
