Applicaton for Certificate of Tax Payment

Outline

This application form is used for certificate of tax payment of the municipal / prefectural residents tax, fixed property tax, city planning tax, corporate inhabitant tax and light vehicle tax ( including vehicle inspection test use). 

How to get the application form

There is the application form at Tax Collection Division of Zama City Hall or please click the link below to download the form.

Fee

300 yen per issue

※It's free for vehicle inspection test use 

What to bring

Valid Identity verification card / document

※Please bring one photo identification which has NOT expired. If you have identification 
   without your photo, please bring two valid identity verification cards / documents. 

How to apply by mail

Please send the following items to the Tax Collection Division.
 

 (1)Completed Application form

 (2)Photocopy of valid identity verification card / document ( See above)

 (3)Postal money order covering exact amount of the fee

   (4)Self-addressed stamped envelope

   ※If you need the certificate of tax payment for vehicle inspection test use, only 
      completed application form and self-addressed stamped envelope are required.   

Notes

  • For us to verify your identity, please provide a valid photo identification when applicants apply at the Tax Collection Division.
  • When you have an agent to apply instead of you or your family members of the same household, the letter of attorney is required. ( For vehicle inspection test use, the letter of attorney isn't necessary.) 
  • Please put the corporate representative's seal on the application form to apply for the certificate of tax payment in corporation name.
  • When you have an agent to apply instead of the corporate representative for the certificate of tax payment in corporation name, the letter of attorney with the seal of corporate representative is required.
  • It may take 7-10 days to confirm the tax payment. If you need it urgently, please bring the receipts. ( If you apply by mail, please enclose the receipts.)
